The operator was fined $ 16,000 on September 2 after a state inspection in June found that 21 out of 25 dialysis units were visibly soiled with “blood splatters, dust and dirt.”

The center was also cited but not fined in 2019 when inspectors found similar filthy conditions at seven out of nine dialysis stations. Quotes issued in 2018 included five cases where staff performed dialysis without following doctor’s instructions, which posed a “high potential risk” of “complications, hospital admissions and death,” according to an inspection report.

The management could not be reached for a comment. A receptionist forwarded the request for comment to the operations manager, who forwarded it to a PR person. The PR person didn’t answer the phone and had a full voicemail box.

TUCSON CENTRAL DIALYSIS (operation as DaVita dialysis)

The center was fined $ 3,000 on April 22 after staff inserted needles into two patients without properly disinfecting the venous access area, a repeat offense. A $ 250 fine was imposed in early 2020 when the same thing happened to three other patients, records show. The center was also charged with failing to keep drug vials under lock and key.
